Jin Hyeok est un des meilleurs neurochirurgiens de Corée, c'est un homme strict qui ne supporte pas l'échec. Il se réveille à l'époque Choseon, où la technique médicale est quasi inexistante. Il devra alors s'adapter et soigner avec les moyens de l'époque. Il sera souvent aidé par Yeong Rae, jeune femme intéressée par la médecine et fascinée par le docteur. Elle ressemble comme deux gouttes d'eau à Mi Na, sa fiancée, et est l'amie d'enfance de Kim Gyeong Tak, un officier militaire qui compte bien l'épouser, et qui voit d'un très mauvais œil, le rapprochement entre la jeune femme et le médecin...
