Cette adaptation développe une sixième route inédite car non développée dans le jeu. L'histoire se déroule dans la ville fictive de Kazamatsuri, où la végétation est omniprésente. En effet, suite à différentes politiques de boisement, la végétation a proliféré donnant à la ville une morphologie unique mêlant arbres et architecture traditionnelle. Kotarô Tennôji, lycéen à Kazamatsuri, possède la faculté d'adapter (rewrite) son propre corps. Il peut ainsi devenir plus fort ou plus agile selon sa volonté. Membre du club d'occultisme de son lycée, il enquête sur les phénomènes paranormaux en compagnie des autres membres, majoritairement des filles.
