Épisode 1 - Aircraft hits PERSON on runway!

A horrible incident happened yesterday 08/05/2020 in Austin, Texas, USA. A Boeing 737-700 from Southwest airlines reported that they had seen, and possibly hit, a person on the runway. The airport sent out a medic vehicle who found a deceased male person on the runway. The involved aircraft received damaged to the left hand engine-nacelle but there were no further injuries to passengers and crew. In this video I want to explain a bit about the options we, the pilots, have in order to avoid runway collisions and obstacles. My thoughts goes out to the involved person, crew and the friends and family of the deceased.

That is exactly what happened on Smartwings flight 1125 on the 22nd of August 2019. The final report has just been released and it is a bombshell of information about the decision making and handling by the involved pilots. An Airbus A380 suffers a catastrophic uncontained engine failure on one of its 4 RollsRoyce engines. This sets off a chain of events that could have easily cascaded into a full blown catastrophe if it wasn't for the cool, professional and knowledgable handling by the 5 pilots and 24 cabin-crew onboard. This is a masterclass in CRM and how things should be handled when sudden technical failures appears on the flightdeck. This is the story of Qantas flight 32 and all the little details that you need to know without the dramatic music of air-crash investigations. On the 19th of July 1989 a United Airlines DC10 suffered a catastrophic engine failure to its tail mounted engine (eng nr2). The engine failure managed to damage all three hydraulic systems on the aircraft rendering completely without normal flight controls. What followed after that is one of the biggest miracles in modern aviation as the 3 person cockpit crew, with the help of a training captain flying as a passenger, managed to get the aircraft down to Sioux City airport. There was still considerable loss of life but 185 persons were saved by the skill, CRM and nerves of the flight and cabin-crew onboard. In this episode I will tell the tale based on my background as a pilot and what I think is important to point out. On the 28th of April 1988 an Aloha Airlines Boeing 737-200 took of from Hilo Airport in Hawaii for a routine short flight over to Honolulu. This flight turned out to be anything but normal. As the aircraft reached its cruising altitude of 24000 feet, the entire front section of the roof in the aircraft passenger cabin suddenly broke loose. In this episode I will give you a full synopsis of the accident, including my own view as a pilot on how the crew handled it and in the second half of the video I will explain why it happened and all technical aspects of this incident.

On the 14th Of August 2005 Helios Airways flight 522 took off from Larnaca, Cyprus on a routine flight via Athens to Prague. Due to a combination a of faulty cockpit setup and misidentification of the different cockpit alarms the pilots became unconscious and the aircraft continued it silent flight towards Athens without anyone at the controls. In this video I will explain the final report and give you my view of the different things that happened on this very tragic flight.

The flight is operated without any fuel gages and, due to a crew calculation error, without much fuel in the tanks. The incredible work by captain Chesley Sullenberger and Jeffery Skiles as well as the heroic work of their cabin crew and first responders led to the successful outcome. On the 26th of June, 1988 a two day old Airbus A320 crashed in a spectacular way during an Airshow in Mulhouse-Habsheim airport outside of Basle, France. Air France flight 296 was one of the first ever passenger flights with the new Airbus A320 and Air France had allowed it to take part in this air-show as part of a publicity day with the new aircraft. During the fly-by at Habsheim airfield, the aircraft seemed unable to climb and ended up flying straight into the forrest on the far side of runway 34. In this episode of "Air Accidents explained" I will dig deep into what happened on this day and why the aircraft seemed unable to follow the pilots commands.

On the 17th of January 2008 a Boeing 777 from British Airways, flight 38, was flying from Beijing, China to London Heathrow, United Kingdom. On final approach to runway 27L, the aircraft lost thrust on both its Rolls Royce RB211 Trent 895-17 engines and the pilots were unable to regain thrust. The aircraft crashed down 330m before the runway threshold and the right main gear separated, the left got pushed through the wing and the nose-gear collapsed. Of the 136 passengers and 16 crew, everybody survived the accident with one serious injury and several minor ones. A true miracle! In this video, I will cover the causes behind this accident and what we can learn from it.

On the 29th of April, 2013 a Boeing 747 from National Air Cargo, flight 102, crashed just seconds after takeoff. The crash killed all 7 American crew members and was captured on a dash-cam of a passing vehicle. On the 12th of June, 1972 , American Airlines flight 96, a McDonnel Douglas DC10-10, suffered a sudden depressurisation when its rear cargo-door detached inflight over Windsor, Ontario. The only technical issue found on the aircraft had to do with the Auto Thrust system but that was a fairly minor problem. What caused this aircraft to crash? Aeroflot Flight 593 was a regular passenger flight from Sheremetyevo International Airport, Moscow, Russia, to Kai Tak Airport in Hong Kong. On 23 March 1994, the aircraft operating the route, an Airbus A310-304 flown by Aeroflot, crashed into the Kuznetsk Alatau mountain range in Kemerovo Oblast, killing all 63 passengers and 12 crew members on board. The investigators found, to their horror, that the children of the pilot was present in the cockpit when the aircraft crashed. In this video I will tell you the full story about what happened.

The Linate Airport disaster occurred at Linate Airport in Milan, Italy on the morning of Monday, 8 October 2001. Scandinavian Airlines System Flight 686, a McDonnell Douglas MD-87 airliner carrying 110 people bound for Copenhagen, Denmark, collided on take-off with a Cessna Citation CJ2. business jet carrying four people bound for Paris, France. All 114 people on both aircraft were killed, as well as four people on the ground.
