L’histoire se déroule dans un futur relativement proche sur une terre ravagée par la guerre nucléaire qui a eu pour conséquence l’évaporation de la plupart des océans et la destruction d’une grande partie de la végétation. Dans cet univers post-apocalyptique, les survivants sont soit d’humbles villageois essayant de survivre, soit des bandits vicieux regroupés en gangs qui s’adonnent aux pillages et à la persécution de ces villageois. Cependant, un artiste martial nommé Kenshiro (Ken), un homme reconnaissable aux sept cicatrices qu’il porte sur le torse, est choisi pour devenir le successeur du légendaire art assassin le Hokuto Shinken (北斗神拳, l’art divin de l’Étoile du nord). Au début de l’aventure, Kenshiro ne cherche pas réellement à aider les villageois, mais au fur et à mesure que son étoile le guide, il se révèle comme étant le sauveur tant attendu par une population au bord du désespoir. Accompagné dans son périple par deux jeunes enfants nommés Bart et Lynn, Ken affrontera un grand nombre de gangs, allant même jusqu'à affronter ses frères adoptifs, eux aussi disciples de l’art du Hokuto Shinken. Ken se mesurera aussi à certains maitres de l’école Nanto Seiken (南斗聖拳, Le poing sacré de l’étoile du Sud).

Kaioh schemes to bring the main family of the North Star to an end. Right in the middle of the funeral for Sayaka being held in Kaioh's castle, an Asura named Gyoko's battle train crashes in. Gyoko, rolling in front of Kaioh, bears a message from Kenshiro: I am never defeated twice! Hyoh, believing that Kenshiro killed Sayaka, is consumed in powerful hatred upon hearing that, and takes his first step towards the dark world...

Sanga, le seigneur de Lastland, règne en tyran sur son enclave désertique. Il assoit sont pouvoir sur le contrôle total des sources d'eau potable de la région, mais aussi sur la présence d'un dieu factice, qui lui permet de fanatiser la population. Mais cela ne lui suffit pas. Il convoite celle que l'on nomme la "faiseuse de miracles" afin de faire d'elle une nouvelle déesse... il compte ainsi obtenir le pouvoir absolu et éternel. Kenshirô décide de sauver les faux dieux de leur prison de pierre, et de délivrer le peuple du mensonge.

Au terme du combat, Kenshirô découvre le secret qu'ils cherchent à tout prix a dissimuler, et perçoit à nouveau le fardeau qui pèse sur les héritiers du Hokuto. Pendant son absence, un homme mystérieux, Seiji, arrive en ville. Consumé par une haine féroce, il vient pour tuer Sanga et prendre sa place sur le trône.

Mais la ville assiégée est désormais à feu et à sang, ravagée par les combats incessants qui opposent les fidèles du dieu Dôha et les sbires de Seiji, retranchés dans leur forteresse de pierre. C'est Tobby qui est à la tête de cette rebellion sanglante. il clame haut et fort son appartenance divine, se targuant d'être le nouveau grand prêtre de la citadelle. et pour asseoir son nouvel Ordre Divin, il n'hésitera pas à déclencher une terrible guerre sainte, qui noiera l'enclave dans les larmes et le sang.
