Black Jack est un médecin sans diplôme au passé trouble et mystérieux. Il travaille avec sa petite assistante Pinoko (qui a un énorme béguin pour lui), traitant des cas médicaux peu connus, qui peuvent être étranges, dangereux ou inconnus. Mais c'est un génie, et il peut sauver la vie de presque n'importe lequel de ses patients (tant qu'ils ont l'argent pour cela), et sa renommée est connue de quelque-uns dans le mode entier, en particulier dans le monde médical. Il est lui-même un homme de science et ne croit pas grand-chose jusqu'à ce qu'il l'ait vu, mais il est souvent surpris par l'amour et la nature qui dominent souvent la science sur laquelle il fonde sa vie.

Dr. Shiitake has the same 20 years of service as the other two candidates, but because he is a low-key person who is treated like an assistant, no one recommended him. Meanwhile, a collapse occurs at a construction site in the city. Black Jack happens to be nearby, and Shiitake, who is on his way home, provides first aid to the injured at the scene. Black Jack is surprised to learn that Shiitake, who has advanced medical skills, is just a regular member of the medical staff at Manaka Hospital.
